Hospitals lose calls in two situations: when the front desk is overloaded, and when it is closed. An AI voice agent covers both. It answers after a set number of rings or outside desk hours, books and reschedules appointments, answers questions about timings, doctors and directions, takes messages, and hands anything urgent to the on-call team. A multi-branch hospital we work with added this "night and overflow" cover alongside its outbound follow-up calls. This is the AI receptionist for medical clinics in practice.

Why calls get missed
- Peak hours: mornings and early evenings, when several patients call at once.
- After hours: evenings, nights and Sundays, when people finally have time to call.
- Lunch and shift changes: short gaps when no one picks up.
A patient who can't get through often calls another hospital. Unlike a missed chat, a missed call leaves no trace unless someone checks the call log.
What the AI handles
| Caller need | What the agent does |
|---|---|
| Book an appointment | Checks free slots and books, sends WhatsApp/SMS confirmation |
| Reschedule or cancel | Finds the booking and changes it |
| Doctor availability and OPD timings | Answers from the hospital's schedule |
| Directions and parking | Gives the branch address and landmarks, sends a location pin |
| Package or procedure enquiry | Gives approved information, takes details for a callback |
| Report or billing questions | Takes a message for the morning team |
| Anything urgent | Gives the emergency number or transfers to the on-call team |
The escalation rules come first
Before anything else, write down what the agent must escalate and how:
- Emergencies: symptoms like chest pain, heavy bleeding or breathing difficulty. The agent immediately gives the emergency number or transfers, and does not continue the booking flow.
- Clinical questions: always handed to a person or a callback, never answered by the agent.
- Complaints: captured in the caller's words, flagged for the morning.
The agent can transfer to a person with a short announcement. Test every transfer route before go-live; see AI-to-human call transfer failures and fixes.
Setting up overflow routing
- During desk hours: ring the desk first; if unanswered after a few rings, route to the AI.
- After hours: route every call to the AI, with the on-call number as the escalation.
- Branch-specific: each branch can have its own timings, doctors and language default.
Languages
A hospital in Bengaluru may want Kannada first; one in Chennai, Tamil; in Hyderabad, Telugu. The agent can default per branch and switch when the caller speaks another language, including Hindi and English.
The greeting
Namaskara, this is the automated assistant for {{hospital}}. This call is recorded. I can help you book an appointment or answer questions about timings. If this is an emergency, please say "emergency" and I will give you the emergency number right away.
Putting the recording notice and emergency path in the greeting avoids problems later. For recording consent and data handling, see DPDP compliance for voice AI.
What the morning team gets
Every overnight call is logged with the outcome, a summary and the transcript: bookings made, messages left, callbacks requested. The morning team starts with a list, not a missed-call log.
Cost
On Edesy, calls are billed per started minute at Rs 4-6 per minute depending on your prepaid top-up. Night and overflow calls are usually short: a booking or a timing question takes one to two minutes.
Start with nights
Most hospitals start with after-hours only, then add overflow during peak hours once they trust the agent. Create a free account, or talk to us about connecting your hospital number. Also read how a hospital uses AI for follow-up calls and 20 questions clinics ask before buying an AI receptionist.
