If you run a hospital, clinic, diagnostic lab or pharmacy that takes interns, you will be asked for a completion certificate several times a year — and there is no single format handed down for it. This post gives you five ready formats, one for each kind of intern you are likely to certify.

Before the samples: what this certificate is, and is not
This matters more in healthcare than anywhere else, so it is worth being direct about it.
A certificate you issue as a hospital is your own document about your own intern. It records that a named person completed a supervised posting with you, on which dates, in which departments, and how they conducted themselves. That is genuinely useful — for the intern's college file, for a future employer, for their own records.
What it is not is a credential. An internship completion certificate used for registration with the National Medical Commission, a State Medical Council, a nursing council or a pharmacy council has to come from the recognised institution through its official process, against its own records. Nothing you format yourself substitutes for that, and no certificate should imply otherwise. Keep words like "verified", "approved" and "valid for registration" off the page entirely.
With that said, here are the formats.
1. MBBS internship certificate (rotatory postings)
The defining feature here is the rotation list. An MBBS intern moves through several departments, and the certificate should name each one with its own dates rather than collapsing them into a single line.

2. Nursing internship certificate (ward postings)
Same structure, but postings are wards rather than clinical departments, and the duties line matters — a nursing college reviewer is looking for what the intern was actually permitted to do.
NURSING INTERNSHIP CERTIFICATE
This is to certify that Ms. Reshma Thomas, a student of College of Nursing, Thrissur, pursuing B.Sc Nursing, has successfully completed the nursing internship at Holy Cross Hospital from 1 January 2026 to 30 June 2026, a duration of 6 months.
Ward postings completed: Medical Ward, Surgical Ward, Intensive Care Unit, Labour Room, Paediatric Ward and Out-Patient Department.
She carried out assigned nursing duties including patient assessment, medication administration under supervision, and documentation. Her attendance, conduct and clinical performance were found to be Excellent.

3. Pharmacy internship certificate (hospital or retail)
For B.Pharm and D.Pharm interns, name the sections — dispensing, in-patient pharmacy, stores, drug information — and mention record maintenance, because that is the part a regulator-facing employer cares about.
PHARMACY INTERNSHIP CERTIFICATE
This is to certify that Mr. Aditya Sharma, a student of Delhi Institute of Pharmaceutical Sciences, pursuing B.Pharm, has successfully completed the pharmacy internship at Metro Care Hospital & Pharmacy from 15 May 2026 to 15 July 2026, a duration of 2 months.
Sections covered during the internship: dispensing, in-patient pharmacy, stores and inventory, and drug information.
He was trained in prescription handling, stock management and record maintenance as per statutory requirements, and his performance was found to be Very Good.

4. Paramedical / DMLT internship certificate (diagnostic lab)
Diagnostic centres issue more of these than they expect. The useful detail is the laboratory sections and an explicit line about safety protocol, since that is what a lab hiring manager reads for.

This is to certify that Ms. Fatima Begum, a student of Deccan Paramedical College, pursuing DMLT, has successfully completed the laboratory internship at Precision Diagnostics Pvt. Ltd. from 1 April 2026 to 28 June 2026, a duration of 2 months and 4 weeks.
Laboratory sections covered: haematology, clinical biochemistry, microbiology, sample collection and reporting.
She followed laboratory safety protocols, handled specimens under supervision and assisted in routine testing. Her conduct and performance were found to be Good.

5. Short hospital training certificate (observership or short attachment)
Not every attachment is a full internship. For a two- or four-week observership, keep it short and say plainly what the person did — and did not — do.
HOSPITAL TRAINING CERTIFICATE
This is to certify that Mr. Karan Joshi, a student of Sinhgad College of Allied Health Sciences, has successfully undergone hospital training at Ashwini Multispeciality Hospital from 1 June 2026 to 28 June 2026, a duration of 4 weeks.
Departments covered during the training: Radiology, Physiotherapy and Medical Records.
The training was observational in nature, conducted under staff supervision. His attendance and conduct were found to be satisfactory.

What every medical internship certificate must carry
Whichever format you use, the certificate should include:
- Hospital, clinic, laboratory or pharmacy name and address, on the official letterhead
- A title naming the internship type
- Intern name, college, course and the college roll or enrolment number
- Each department, ward or section posting, with its own dates
- Total duration of the internship
- A conduct and performance remark
- Reference number and issue date
- Signature of the authorised signatory, with the institution's seal
- A guide, unit head or HOD counter-signature where your institution requires one
The two that get missed most often are the enrolment number and per-rotation dates. Both are the usual reason a college sends a certificate back.
Issuing more than one at a time
